Autoimmune conditions have very high rates of anxiety, including autoimmune thyroid conditions [R, R]. Can natural anxiety and pain remedies help both situations? There are several kinds of anxiety, including Generalized Anxiety Disorder (GAD), Panic Disorder (PD), Social Anxiety Disorder, Specific Phobias, Obsessive-Compulsive Disorder (OCD), Posttraumatic Stress Disorder, and Major Depressive Disorder (MDD) [R].
All forms of anxiety can cause personal, family, and social impairments [R].
Conventional medication for GAD can include benzodiazepines, antidepressants, and anticonvulsants. However, all of these drug treatments can have challenging side effects, including addiction, abuse, amnesia, slow response, sexual dysfunction, sedation, and/or dizziness [R].
The Thyroid-Anxiety Connection
People with autoimmune thyroiditis disorders are over twice as likely to also suffer from anxiety than the general population [R].
Anxiety in other autoimmune conditions is very common and can be debilitating, such as in autoimmune hepatitis [R]. Both depression and severe anxiety are quite common in this case.
Stress is a known trigger for many diseases, and thyroid tissue damage due to stress is no exception [R].

Copaiba for Anxiety
Copaiba benefits the body and mind by reducing inflammation. In a recent research study, copaiba oil reduced anxiety-like behaviors in a maze study of rats [R]. The effect of copaiba oil was dose-dependent; the higher the dose, the bigger the reduction in anxiety behaviors was seen.
Copaiba’s main compound, B-caryophyllene, at a dose of 50 mg/kg, was able to reduce anxiety, compulsive, and depression-like symptoms in a maze and forced swim test in mice [R].
While clinical studies are still needed for the specific purpose of anxiety, its benefits for the neurons, and protection from inflammation support that it may help this condition [R].
CBD Oil for Anxiety
In addition to the CBD’s effects on serotonin, CBD may rapidly reduce depression symptoms by increasing brain-derived neurotrophic factor (BDNF) signaling as shown in preclinical work [R].
A small clinical study found that CBD single dose at 400 mg was better than a placebo at reducing signs of social anxiety due to its positive effects on the limbic and paralimbic parts of the brain (R).
CBD was also able to reduce the effects of fear memory over a 48-hour period in a small clinical study. The dose used in this study was 32 mg (R).
It should be noted that cannabis WITH THC can increase anxiety symptoms, however (R). Make sure your patients’ CBD comes from hemp seed and not marijuana plants.
What are Endocannabinoids?
An appealing and emerging area for anxiety treatment involves the use of endocannabinoid plants. These include many herbs, like lavender and hops, spices, kava root, copaiba resin, cannabidiol (CBD), marijuana, chocolate, and black pepper (R, R).
Endocannabinoids are chemical messengers. The endocannabinoid system is a complex lipid signaling system where different proteins control or modulate numerous processes that tell our body to start different processes and when to stop. For this reason, they play key roles in homeostasis in the body (R).
We have receptors for endocannabinoids throughout the body called cannabinoid receptors. Some cannabinoid receptors are highly concentrated in regions like the central nervous system; these are called CB1 receptors.
CB2 receptors are common in the endocrine system, immune cells, gastrointestinal cells, brain, and in the peripheral nervous system. Some conventional drugs work through these receptors to relieve pain, including acetaminophen (R).
What is Beta-caryophyllene?
Beta-caryophyllene is a sesquiterpene and is the most well-known endocannabinoid compound from plants. Beta-caryophyllene specifically binds CB2 receptors and thereby reduces inflammation, and pain, and even may help protect organs such as kidneys and liver in autoimmune diseases. β-Caryophyllene is a CB2 receptor agonist (R).
Many people find that plants containing beta-caryophyllene help reduce anxiety and depression. Preclinical work shows that beta-caryophyllene extract exerts anxiety-reducing effects (R).
What is the evidence for this?
The most commonly used plants for their beta-caryophyllene content are cannabinoid oil from marijuana and copaiba essential oil. The plant with the highest known beta-caryophyllene content is copaiba essential oil (R).

What is Copaiba?
Copaiba pronunciation is kow·pai·ee·buh.
Copaiba is an oleoresin that comes from the Copaifera genus. It is a tree that grows in northern South America and is closely related to the black pepper tree. The resin, also known as balsam, is extracted from trees for their medicinal uses (R).
There are a variety of copaiba species including Copaifera martii, Copaifera officinalis, and Copaifera reticulata, and Copaifera langsdorffii (R).
Copaiba essential oil is a distilled resin from the copaiba tree. Other names for copaiba include copaiba balsam, copaiba olie, oleo de copaiba, or balsam essential oil (R).
Copaiba balsam is the same as copaiba. Both copaiba balsam essential oil, balsam oil, and copaiba oil are used medicinally (R). Copaiba balsam oil is not to be confused with balsam of Peru or balsam fir; these are different species with different properties.
The common method of extraction and preferred method is steam distillation (R).
The scent of copaiba oil is subtly sweet, peppery, and woody. Copaiba is found as an ingredient in foods, soaps, perfumes, and cosmetics. Copaiba is used to make natural pharmaceutical medications as well (R). It has a base perfumery note.
Copaiba Traditional Uses
Traditional copaiba use began when humans first observed animals using copaiba to treat wounds (R). European settlers observed native peoples using the copaiba resin on their babies. The biological properties of copaiba are related to its diterpenes and sesquiterpenes (R). Some of the sesquiterpenes in copaiba include β-caryophyllene, α-copaene, zingiberene, β-bisabolene, and bergamotene. Some of the diterpenes in copaiba are kaurenoic, hardwickiic, kovalenic, polyalthic, and copalic acids (R).
More Copaiba Essential Oil Uses
The research studies backing the use of copaiba for health conditions are emerging, and include possible treatment for infections, anxiety, nerve issues, pain, and more.
Copaiba May Protect Neurons
A single oral dose of copaiba (Copaifera reticulata) reduced nerve damage and protected neurons by reducing neutrophil recruitment and microglia and macrophage activation in rats (R). Damage to the motor cortex was decreased by almost 40 percent.
Why is this important? Overactivated macrophage and microglia cause tissue injury following central nervous system in humans (R).
Another study showed that copaiba benefits include anti-inflammatory effects in the central nervous system in rats (R). Because of this, copaiba could help with numerous brain disorders, including autism, memory loss, neurodegenerative conditions, multiple sclerosis, and more.
Copaiba for Pain
Chronic pain in autoimmune diseases can cause a lot of anxiety. One way that copaiba can help the body deal with anxiety is by dampening the pain severity in the body. With copaiba’s anti-inflammatory and antioxidant mechanisms, it can be a useful tool for dealing with pain and anxiety (R).
How does copaiba for pain work? Copaiba reduces inflammation due to autoimmune disease in a rat model of autoimmune encephalitis. How? By reducing NF-κB translocation, and consequently the secretion of some cytokines, such as IL-6 and TNF-α (R).
A blend of copaiba oil with Deep Blue rub was effective at reducing arthritic pain in a clinical study; there was a 50% decrease in total pain scores, increased finger strength, and increased angle of maximum flexion compared to placebo (R).
Beta-caryophyllene in copaiba is a full agonist, not partial, of CB2, which is a striking finding.
Oral administration of this dietary compound creates a potent anti-inflammatory effect in mice, but not in CB2 knockout mice. Beta-caryophyllene also inhibits the pathways triggered by activation of the toll-like receptor complex. This complex results in an expression of proinflammatory cytokines, including IL-1β, IL-6, IL-8, and TNFalpha. It also promotes the TH1 inflammatory immune response (R).
Copaiba for Infection Treatments
Copaiba has antimicrobial effects on bacteria by disrupting the cell wall of these microorganisms. Copaiba selectively is antimicrobial against gram-positive bacteria, including Staphylococcus aureus, methicillin-resistant S. aureus, Staphylococcus epidermidis, Bacillus subtilis, and Enterococcus faecalis in cell studies (R, R).
Copaiba benefits the skin by reducing acne, according to a small, randomized placebo-controlled trial (R). Copaiba reduces dental biofilms as well (R).
Copaiba oil also has antiparasitic effects as well as antifungal effects according to cell studies against the following: Leishmania amazonensis parasite (R).
The number of cases of leishmaniasis is on the rise on all continents. It is considered is endemic in 88 countries in Africa, Asia, Europe, and North and South America, according to the World Health Organization (R).
Mouse study also finds copaiba effective against parasites: copaiba resin oil showed antiparasitic activity against the leishmaniasis called L. amazonensis (R).
Copaiba Oil May Reduce Cancer
Both cell studies and animal studies suggest that copaiba may help reduce cancer growth and spread. For example, a mouse study showed that copaiba reduced lung tumor growth by 58% and tumor weight by 76%. Lung nodules were reduced in number by 47% as well (R).

Copaiba May Help Heal Wounds
Copaiba essential oil diluted to 4% and applied topically to open wounds sped up the healing process by 84% compared to 51% at day 9 in a rat study (R). The tensile strength also improved more quickly in the wound area with the use of copaiba oil compared to controls.
More work is needed to determine if copaiba oil helps with wound healing. Copaiba’s antimicrobial effects and effects on the speed of healing in early research shows great promise in this area.
How to Use Copaiba Essential Oil
You can use Copaiba topically, diffused, and internally if you get Certified Pure Therapeutic Grade (CPTG) essential oils. doTerra copaiba is one such oil.
Some websites warn against essential oils for internal use, but this is because many brands contain impurities and solvents. CPTG copaiba oil is proven safe for internal use when used according to manufacturer directions.

What is CBD Oil?
Pure cannabidiol (CBD) oil comes from the cannabis sativa plant (hemp) and it is extracted from the resin of cannabis buds and flowers that are diluted with another oil, like medium-chain triglyceride (MCT) oil. CBD is the active compound for CB2 receptors derived from marijuana (R).

In 1839, an Irish physician and medical researcher named William O’Shaughnessy investigated the therapeutic effects of cannabis. O’Shaughnessy research opened doors to the discovery of the compounds in cannabis referred to as cannabinoids (R).
Early research on cannabinoid plant structure wasn’t discovered until 1963 when Dr, Raphael Mechaoulam revealed the direct relationship of the euphoric effects of THC and the mind-altering compound of CBD (R).
A Note About CBD Research
Cannabis as marijuana is still considered a Schedule 1 drug, or illegal, in most states. For this reason, research has been frustratingly slow to emerge for pure CBD oil. The last few years have given us a lot of insight, but we still have a ways to go.
Hemp Oil vs CBD Oil
As healthcare providers, it is important to know the differences between hemp oil and CBD oil. There are a lot of mixed messages, both in research and on posts.

Hemp oil is used in cosmetics and as a food that provides rich omega-3 fatty acids, omega-6 fatty acids, and gamma-linolenic acid (R).
Hemp oil should be CBD and THC-free.
Hemp oil is extracted from hemp seed. In contrast, CBD is primarily extracted from hemp plant stems. The hemp plant can be specifically cultivated to be high in CBD and only contains trace amounts of THC. This makes the ratio of CBD to THC much higher (R).
CBD Health Benefits
CBD works directly on the brain receptors 5HTIA (serotonin) and GABA to lower anxiety and depression (R). CBD lowers inflammation by suppressing cytokine production and inducing T-regulatory cells to protect the body (R).
CBD can decrease chronic pain because it inhibits the transmission of neuronal signaling through pain pathways.
CBD digestion benefits include the following: it helps heal the leaky gut that can contribute to intestinal permeability and decrease spasmodic activity that is common with irritable bowel syndrome (R).
Using CBD for Seizures
In a study by Mechaoulam, 300mg of CBD was administered to 8 subjects for 4 months (R). During that time frame, half of the subjects had a decrease in seizures. This was a huge breakthrough and changed the lives of more than 50 million epilepsy sufferers in the world.
Seizure activity was reduced with the use of CBD oil in both rat and mouse models of temporal lobe seizures (R). CBD improved memory and motor function and reduced working memory errors due to seizure activity in this study as well.
Cbd Oil for Autoimmune Conditions
Preclinical work suggests that CBD oil may help reduce autoimmune disease by increasing myeloid-derived suppressor cells (MDSCs). These types of cells are involved in the regulation of T cells (R).
Other preclinical work shows that CBD may suppress immune cells, which can be an important effect on autoimmune disorders. This means that CBD oil likely has important immune system effects that will be explored with further research.
Cbd Oil and Autism Symptoms
Pure CBD oil was helpful for symptoms of autism in a small open-label trial of CBD oil in children. Hyperactivity symptoms improved in 68% of children, sleep problems improved in 71%, anxiety improved in 47%, and worsened in 24% of the group. Adverse effects were generally mild.
An observational study of children with autism in a real-world study found that parent-reported anxiety symptoms were reduced in 88% of children using CBD oil (R). Symptoms such as restlessness improved in almost 90% of the children, indicating CBD oil for ADHD may also prove beneficial.
While no randomized, double-blinded trials have been conducted, anxiety symptoms in most children with autism appear to improve with CBD oil with the research we have so far.

Cannabis has proven effective in numerous kinds of pain settings as well, including peripheral neuropathic pain, rheumatoid arthritis, and cancer pain (R). However, when looking at isolated CBD data, the research gets a little more sparse because the largest pain-relieving substance in cannabis is THC.
Preclinical work is promising for isolated CBD: transdermal cannabidiol reduces inflammation and arthritis in rats (R). CBD also reduced nerve damage in rats with osteoarthritis (R).
A lot of people are seeing variable results for pain relief with CBD oil, and this is likely due to the form, concentration, type of pain being treated, and duration of use.
CBD Oil for Migraines
Many CBD testimonials support the use of pure CBD oil for migraines. However, no clinical research has looked specifically at CBD use for this condition.
According to Medical News Today, it is a viable option for patients to try if they haven’t had relief from other therapies.
CBD May Reduce Cancer
CBD helps reduce the breakdown of a beneficial compound called anandamide (R).

Concentrates: Concentrates are like tinctures, however, concentrates use a much stronger dose of CBD. The high potency is not recommended for people taking CBD for the first time. Recommendations are to start at a low dosage and work up.
Capsules: Capsules or pill form can be easily taken with water and can be very convenient, however, the doses typically contain 10-25 milligrams of CBD. Make sure to know what dosage you need and can handle before taking by starting at a low dose of CBD.
Topical Solutions: Topic options include lotions, salves, and lip balms. They are primarily used to benefit the skin, joints, and muscle pain and work into the dermal layers of your skin. CBD patches are also available to be delivered directly into the bloodstream.

CBD Oil Recipes
The following recipes are examples of how you can add CBD oil to everyday meals. They are from the website Cannabinoid Creations.
CBD Pesto Pasta
- 2 Cups of fresh basil leaves
- 3 garlic cloves
- ½ cup of pine nuts or walnuts
- ¾ cup of grated cheese
- ½ cup of CBD oil
Directions:
Put basil, garlic, and pine nuts, into a food processor and pulse for 2 mins or until you get a coarse paste. Transfer to a bowl to add oil and cheese, then stir. Toss Pesto sauce into a bowl of cooked pasta.
CBD Chocolate Chip Cookies
- 3 cups gluten-free flour
- 1 teaspoon of baking soda
- 1 cup of monk fruit sweetener
- 1 cup of unrefined coconut sugar
- ½ teaspoon of salt
- 1 cup of butter
- 2 cups of dark chocolate chips
- 2 eggs
- 2 teaspoons of hot water
- 2 teaspoons of vanilla extract
- 0.5 oz of unflavored pure CBD oil
Directions:
Preheat your oven to 350 degrees. Mix oil with softened butter, brown sugar, and white sugar. Mix until smooth, add eggs one at a time. Stir in the vanilla extract. In a separate bowl.

- 10 drops lavender oil

Risks, Sided Effects & Contraindications
This section will go over potential risks, side effects, and contraindications of using the essential oil copaiba.
Copaiba Essential Oil Side Effects
Copaiba oil is generally safe for use as above for topical, diffused, and internal use. As with most plants that have medicinal effects, too much can cause some temporary side effects, such as nausea, and abdominal discomfort.
If not diluted properly, it can cause skin irritation. There is a chance that you could also be allergic or sensitive to the oil. It is best to try on a small area of skin first.
Risks of Copaiba
Some concern about liver damage exists in an early animal study (R). Newer research shows that copaiba at high doses has low toxicity (R).
Further, research also shows that copaiba can actually protect the liver from damage due to acetaminophen in a rat study (R).
CBD Side Effects
It’s important to consult your healthcare professional before considering using any CBD products. However, CBD has few adverse side effects when used in the appropriate amounts.
Some common complaints are mild drowsiness, dizziness, diarrhea, nausea, vomiting, dry mouth, anxiety, and mood changes. It is recommended to start at a low dosage and work your way up depending on your health issues.

CBD Oil and Copaiba Medication Interactions
More studies are needed to determine the interactions between CBD oil and medications. Cannabidiol may interact with warfarin, omeprazole, and risperidone medications, but it is too soon to know to what extent the interactions may be (R).
According to Rx List, copaiba may interfere with lithium by decreasing how well the body eliminates lithium (R).
When to Use Copaiba verse CBD
It is impossible to remove trace amounts of THC from pure CBD oil. If your job requires drug testing, you run the risk of testing positive for drugs.
Copaiba contains a higher percentage of beta-caryophyllene; 55 percent versus around 35% in cannabis.
Copaiba oil is also a fair amount more affordable and requires a very small dose; 1-2 drops several times a day can be plenty. At 14 cents a drop, it is a very reasonable option.
In contrast, a single dose of CBD can be over $1.
When to Use CBD over Copaiba
More research supports the use of pure CBD oil over copaiba for seizures. As the legal status has been lifted for CBD, research will likely provide us with a lot more insight into the vast uses of CBD oil.
